New Patient Registration - Walton Village Medical Centre

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

Eligibility can be quickly confirmed from your address - so please provide proof by way of a recent utility bill AND one form of photographic ID (preferable).

Please note: Medical treatment will only be available on completion of eligibility confirmation.

PLEASE ENSURE YOU HAVE ENOUGH REPEAT MEDICATIONS [usually one month supply] FROM YOUR PREVIOUS GP, UNTIL YOU CAN REGISTER WITH US.

PROCEDURE TO REGISTER WITH OUR PRACTICE

  • Step 2 : Bring BOTH the New Patients Medication checklist &  GMS-1 form to our Reception with a utility bill [address proof ]and one photo identity id (preferable).
  • Step 3 : A new patient medical appointment will be made with the practice Nurse within 21 days.

After successful completion of your Step 3, You are considered as registered with our practice and can make appointments to see the doctor.

Please note: It currently takes 6 - 8 weeks for your medical records to arrive. Hence the information you enter on the GMS-1 form is very important.

GMS-1 Form

You will need to complete a (GMS-1) registration form which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients must undergo a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
